UH Manoa presents their COVID-19 Vaccination Clinic for UH students, faculty, and staff only. This is a free walk-in clinic that does not require any appointment. All you need to do is follow the checklist listed on the post and show up on February 2nd from 9:00am to 1:00 pm.
Participants must do the following:
Complete the Pre-Vaccination Assessment and Consent and Demographic and Insurance Information forms (which can be found here: https://drive.google.com/drive/folders/1sGvd7r6__NZvXcnatGHqg6-6ErwEAjUw). A parent/guardian is required to sign for anyone under 18 years of age.
Bring your vaccination card (if you have already received your first and/or second dose).
Bring a valid photo ID.
Bring your insurance card (uninsured participants are also welcome).
Be cleared to come on campus by the LumiSight UH app.
Wear a face mask and practice social distancing.
